Notes - LKH note: MHR gives Martha's death date as 15 April 1917 in Olive Hill Kentucky. But I find her in the 1920 US census, widowed, living with her brother E. R. Phelps, widower, in Harvey, Cook County, Illinois. She is also listed as his closest relative on his 12 September 1918 draft registration, living at his same address.
